Newcastle Hospitals is looking at whether the failure has had any impact on the care and treatment of patients.

The funeral of Faye Fantarrow took place on Thursday 21 September after the Sunderland musician died from a brain tumour.

Michael Brown was left waiting for an ambulance for more than 14 hours before being taken to hospital, where he died with sepsis.
